Description of supplemental materials required
The supplemental application requires students to complete questions related to community standards, residency (if an applicant wishes to claim NC Residency for Tuition Purposes), and military service as mandated by NC state law.  The supplemental application will guide applicants on supporting documentation required and will be sent to each applicant within 3-4 business days after they submit their PTCAS application.   Applicants do not need to list schools attended or provide transcripts through the supplemental application.  Transcripts will be provided only through PTCAS.

Applicants MUST submit GRE scores through the PTCAS GRE code of 7733. Codes submitted to the University of North Carolina-Chapel Hill GRE school code will NOT be accepted.  The UNC Division of Physical Therapy will consider all qualified applicants. Applicants must earn a minimum score of 40% on each section of the GRE.    The noted scores must be obtained within a single testing session.

UNC will use the last four semesters taken as a full-time student (undergraduate and/or graduate) for the overall GPA calculation.  

UNC uses only the highest course grade earned in calculating the program specific prerequisite GPA. If a prerequisite course is repeated, we will use the higher of the two grades.
