Description of supplemental materials required
The supplemental fee is a one-time fee payable to the University of Colorado.  Payment of $100 can be made directly using the weblink address below. This fee is due at the time the application is submitted to PTCAS (deadline is 10/1/2020). No additional written applications or materials are required beyond the completed application through PTCAS. The Supplemental fee MUST be paid in order for your application to be reviewed and considered by our PT Program's Admissions Committee.  

All applicants are REQUIRED to participate in CASPer on one of ten available test dates. The final date for applicants to our program is October 13, 2020.  CASPer is a selection tool used by academic programs to help assess applicants for non-academic attributes or people skills. Registration, test dates and fee information are available at the following link: https://account.takecasper.com/signup. 

We encourage applicants to provide details of all experiences within the PTCAS application form. Do not include a resume.

Obtain three (3) references, including AT LEAST one (1) from each of the following:
    1) Licensed physical therapist whom the applicant has observed working with patients in a PT setting AND who holds a current U.S. PT license.
    2) Science professor or course instructor from the student's major, from a basic science course, or from a course that meets an upper division science prerequisite.
    3) Additional professor, health professional, OR recent employer or supervisor

**Additional information regarding these references:
 - Health Care Professional (for reference type #3) must be licensed as: MD, DO, RN (NP, MSN, BSN, DNP), PA-C, PT (BS, MS, MPT, DPT), or OT (BS, MS, MOT, DOT). 
 - We do NOT accept references from PTAs or OTAs.
 - Reference from a Professor can be completed by an Instructor, Assistant, Associate or Full Professor who was the primary teacher or course coordinator.  
 - We do NOT accept references from guest lecturers, TAs or GAs.

PT Observation Hours

We require a minimum of 15 hours in EACH of 3 settings:
 - Minimum of 15 hours in an inpatient setting: hospital, rehabilitation facility, or extended care facility; AND  
 - Minimum of 15 hours in an outpatient setting: free standing PT clinic or outpatient hospital clinic; AND
 - Minimum of 15 hours in an additional setting: school/preschool, home health, wellness/fitness based practice, industrial/occupational, or other PT specialty care 

NOTE: We encourage quality, reflection, and breadth of observation over quantity of hours. Observation hours are to gain exposure to the roles and responsibilities of physical therapists within different settings and with different patient populations.

Additional information about program's GPA requirements
Program will use the highest grade earned in a repeated course to meet the prerequisite.  However, when calculating the overall GPA, however, ALL recorded grades are included in the average. 

Grades  of C- and below are not accepted in any course that is intended to meet a prerequisite.

Applicants with an Overall Cumulative GPA below the minimum of 3.0 may be considered when there is evidence of a positive trend in grades over time AND a higher GPA in courses identified as program prerequisites.

Non-citizens will need to provide appropriate documentation to be eligible for admission. For additional information, contact International Student & Scholar Services: http://www.ucdenver.edu/academics/internationalprograms/oia/isss/Pages/default.aspx English translation and WES evaluations are required for all original foreign transcripts. Applicants must have a transcript evaluation that states that the foreign Bachelor's degree is equivalent to a Bachelor's degree in the United States to be eligible to apply to this Program. In addition, the applicant must have transcripts and evaluations that meet any additional prerequisite courses not included in the Bachelor's degree evaluation report. Applicants who have attended a Canadian institution that uses a non-traditional or non-conforming grading system, will need to provide a WES evaluation.
