Description of supplemental materials required
Applicants must submit an IU Graduate CAS supplemental application, the Intent to Complete Degree Requirements form, and the IU DPT Prerequisite Course Completion Form, in addition to all PTCAS materials, no later than the application deadline of October 1, 2021.

Applicants who fail to complete both the PTCAS application and the Indiana University Graduate CAS application cannot be considered for the IU DPT program. 

The PTCAS application must be in COMPLETE status at the application deadline.

Document upload requirements
1) Prerequisite Course Completion Form: Enter prerequisite courses completed on fillable form, save the form, then upload and submit in Documents as a Catalog Course Descriptions. 

2) Declaration of Intent to Complete Degree Requirements Form: Enter information and obtain signatures on the form, save the form, then upload and submit in Documents as an Other.  You need to submit this form if you do not currently hold a bachelor's degree by the application deadline. Note: All degree requirements must be completed before the start of classes in May.  All coursework must be completed prior to the start of the program in May. Those applying without a degree will be required to submit a final transcript showing the degree awarded. Applicants holding a bachelor's degree at the time of application may omit this form.

PT Observation Hours
Information about your PT requirements
Applicants must have a minimum of 20 hours in an inpatient setting and 20 hours in an outpatient setting. Please be sure your verifying PT indicates the setting in which you observed.

Program will not accept observation hours with a PT Assistant.

Additional information about program's GPA requirements
A maximum of 15 credits of prerequisite coursework may be retaken with the higher grade used to calculate the math/science prerequisite GPA; all courses will be included in the cumulative GPA calculation.    

Undergraduate cumulative GPA will be calculated from all courses completed with a grade from all undergraduate institutions attended. Prerequisites and baccalaureate degree must be from a regionally accredited institution or international equivalent.

Students are allowed to apply AP credit of scores 3 or higher toward the completion of their prerequisite requirements.  If the AP course is a math/science prerequisite the GPA calculations will be obtained by omitting that specific course.

Students are allowed to apply Dual Credit courses toward the completion of their prerequisite requirements.  An official university transcript showing the grade earned must be sent to PTCAS.
