Upload a personal video of yourself to youtube and include the link to the video  at the top of each document and where requested on the checklist form.  Refer to the PTCAS application for supplemental video questions.

Supplemental application documents include: Admissions Checklist, Resume, Copy of GRE scores (from the Testing Center), completed Prerequisite Worksheet, YouTube link and PTCAS # at the top of each form.
