The DPT Supplemental Prerequisite Worksheet is used to list completed, in-progress, and planned prerequisite courses.  This document must be completed and uploaded to the PTCAS application by the October 3, 2022 deadline. 

Cal State Apply Application.  Only applicants invited to interview are required to submit the Cal State Apply application and pay the $70.00 fee.

Applicants must arrange to have three letters of recommendation sent to PTCAS by the deadline date.  

The first letter of recommendation must be completed by a physical therapists licensed in the U.S. or Canada who has worked with the applicant in a physical therapy setting.  The supervision needs to have occurred when the applicant was either a volunteer or an employee at the clinic. 

The second letter of recommendation must be provided by a college professor (either major or non-major) who has been an instructor of the applicant.

The third letter of recommendation can be from a second licensed physical therapist, a second college professor, or from a supervisor or employer.  
PT Observation Hours

Applicants must complete and verify on PTCAS a minimum 100 total hours of observation/work/volunteer experience under the direct supervision of a licensed physical therapist.  

All 100 PT experience hours must be "COMPLETED" and verified by the October 3, 2022 deadline date.  "PLANNED" hours will not be considered.

All prerequisite courses must be completed by the start of the program with a grade of C or higher.  A minimum 3.00 GPA is required in all of the prerequisite courses to be eligible to apply.  Applicants may have up to 4 prerequisite courses in-progress or pending when they submit their application.  At least 2 of the these courses must be completed by the end of the fall semester, and the remaining 2 courses to be completed the following spring.  

A minimum 3.00 GPA for on all undergraduate coursework is required.  The undergraduate cumulative GPA is calculated by PTCAS.  For additional information on how your GPA is calculated, visit

Contact the CSU, Sacramento Office of Global Education at 916-278-6686 for information regarding transcript evaluation requirements by the university. Send transcripts and transcript evaluations to the Office of Graduate Studies at CSU, Sacramento by December 31, 2022. DOMESTIC students applying with transcripts from outside the U.S. should carefully review instructions at and submit their Cal State Apply Admissions application by December 31, 2022. FOREIGN applicants must have at least 8 units of prerequisite coursework completed prior to December 31, 2022 at a US accredited institution. Applicants who do not possess a bachelor's degree from a post secondary institution where English is the principal language must demonstrate competence in English by scoring a minimum of 550 (paper-based), 213 (computer-based), or 80 (internet-based) on the Test of English as a Foreign Language (TOEFL). Foreign applicants must complete their observation hours in US physical therapy clinics.
